Required Information for Transcripts:

1. Complete Name while attending Liberty High School
2. Date of Birth
3. Year of Graduation or Withdrawal Date
4. Complete address where you would like the transcript to be sent
5. $2 check made out to Liberty High School for each transcript ordered.

Or, you can come into the Liberty High School Counseling Office to pay and order transcripts. Please allow two working days for transcript requests to be processed.
